Our Support at Home services are designed to give you the right help at the right time, so you can continue living independently in the place you know and love. Whether you need a little assistance with daily tasks or more regular support to manage your health and wellbeing, we’re here to make every day easier.

Our Support at Home services cover a wide range of practical, social and clinical supports including:

  • Personal care – showering, grooming, dressing, mobility support.
  • Domestic assistance – cleaning, laundry, meal preparation.
  • Social connection – companionship, outings, community activities.
  • Transport – appointments, shopping, social visits.
  • Nursing care – medication support, wound care, health monitoring.
  • Allied health – physiotherapy, occupational therapy, podiatry.
  • Home maintenance – minor repairs, safety checks, yard care.
  • Assistive technology – guidance, setup, and training.
  • Care coordination – planning, reviews, and ongoing support.

How do I access government subsidised care?

Step 1: Contact My Aged Care

My Aged Care is the Government agency that assesses your needs, and if eligible assigns your package. You can call them on 1800 200 422 or visit their website at My Aged Care.

Step 2: Be assessed over the phone

My Aged Care will guide you through an initial phone assessment to understand your needs and eligibility. They may then refer you to:

  • The Regional Assessment Services (RAS) team – who provide entry level assessments for Commonwealth Home Support Program (CHSP) services and lower level packages.
  • The Aged Care Assessment Team (ACAT) team – who provide a comprehensive assessment if you have more complex needs and require support under Home Care Packages funding.

Step 3: Have an ACAT assessment

An ACAT assessor (nurse, social worker, or health professional) will visit you at home to conduct a thorough assessment. They will talk to you about how you live day-to-day and what you need help with to stay living independently and well at home.

This assessment determines eligibility for:

  • A Home Care Package and at which level
  • Interim CHSP services
  • Other services like respite, transition care or placement in an aged care facility.

It’s helpful to have a family member or support person present during the assessment.

Step 4: Secure your Home Care Package

After your ACAT assessment, you will receive a letter confirming your eligibility and the level of the Home Care Package assigned to you. You will be placed in a national queue and allocated a package as one becomes available.

Step 5: Contact us

After your assessment we encourage you to contact CatholicCare to discuss your Home Care Package status and to enquire whether we have interim CHSP services that might be able to assist you whilst you wait to access to your package funds.
