We are now able to support people who are eligible for a Support at Home program in Northern Sydney or the Central Coast

Our Support at Home (SaH) services are designed to give you the right help at the right time, so you can continue living independently in the place you know and love. Whether you need a little assistance with daily tasks or more regular support to manage your health and wellbeing, we’re here to make every day easier.

They cover a wide range of practical, social and clinical supports including:

  • Personal care – showering, grooming, dressing, mobility support.
  • Domestic assistance – cleaning, laundry, meal preparation.
  • Social connection – companionship, outings, community activities.
  • Transport – appointments, shopping, social visits.
  • Nursing care – medication support, wound care, health monitoring.
  • Allied health – physiotherapy, occupational therapy, podiatry.
  • Home maintenance – minor repairs, safety checks, yard care.
  • Assistive technology – guidance, setup, and training.
  • Care coordination – planning, reviews, and ongoing support.

We know that everyone is unique. That’s why our SaH services are flexible and tailored to your individual needs and goals. You decide the support that’s right for you, when you need it, and how it fits into your day-to-day life. From practical help around the home to social connection, transport, and clinical care, our team is ready to step in with compassion and reliability. Most importantly, you remain in control. We simply walk alongside you, offering the support that helps you stay confident, independent, and connected to the things that matter most.

With clear communication, reliable service, and a warm, human approach, we aim to make every interaction feel supportive and reassuring.

Our staff are trained, experienced and committed to delivering respectful, person centred care. You’ll see familiar faces, receive consistent support, and have a dedicated team who work with you, your family, and your health professionals to ensure your needs are met.

How do I access government subsidised care?

Step 1: Contact MyAgedCare

MyAgedCare is the Government agency that assesses your needs, and if eligible assigns your package. You can call them on 1800 200 422 or visit their website at MyAgedCare.

Step 2: Be assessed

MyAgedCare will guide you through an Initial Assessment Tool (IAT) to understand your needs and eligibility.

Step 3: Eligibility confirmation

You will receive a Notice of Decision. If you are eligible you be approved for a classification 1 to 8 (approx. $11,000 to $78,000 of funding annually) and receive a Support Plan.

Step 4: Choose a provider (CatholicCare is a great option)

Once funding is allocated, you will have 56 days to choose a service provider and enter into a Service Agreement. Your chosen provider reviews the Support Plan, develops a Care Plan and Budget.

We encourage you to contact CatholicCare on 1800 324 924 to discuss your options. We may have interim CHSP services that might be able to assist you whilst you wait to access to your funds.
